Job summary

BlueCross BlueShield of South Carolina is hiring an Intensive Care Management Coordinator to serve as an Intensive Case Manager for members with defined conditions. The role is remote with travel across a South Carolina region, requiring in-person visits and care coordination with families and healthcare providers.

The ideal candidate has clinical Behavioral Health experience or comparable utilization/case management background, plus knowledge of insurance programs and quality improvement

Qualifications

• Bachelor's degree in a health-related field or equivalent combination of education and experience.
• Experience working with behavioral health conditions and health management programs.
• Knowledge of insurance/benefit programs and quality improvement processes.

Responsibilities

• Assess, plan, coordinate, monitor, and advocate for assigned members with in-person visits.
• Collaborate with members, families, caregivers and healthcare professionals involved in care.
• Coordinate health management activities and programs, update procedures as needed.
• Perform in-person assessments in members' residences or approved environments.
• Utilize data systems to document information and analyze reports for program impact.
• Identify opportunities to improve participation and outcomes in health management programs.
