Qualifications

• Access to reliable transportation and ability to travel within the designated area.

Responsibilities

• Utilize advanced clinical skills to engage and motivate consumers.
• Assist consumers and families with connections to appropriate referrals.
• Identify and remove barriers to service delivery success.
