Job summary

OptumCare is seeking a Behavioral Health Care Advocate to help enrollees stay stable in the community and access services for treatment goals. The role acts as a clinical liaison between health plan staff and the clinical network, working 80% in the Buffalo area with hospitals and home visits and 20% remote.

Qualifications

• Master's degree or higher in psychology or related field; NY licensure may be required.
• Active, unrestricted NY license (LMHC/LMFT/LCSW/LP) or eligibility.
• At least 2+ years in a related mental health environment.
• Proficient with Excel and general computer skills; reliable home office setup.
• Valid driver's license and ability to travel within Buffalo area.

Responsibilities

• Identify and link community services to enrollees.
• Assist with aftercare appointments and follow-up.
• Collaborate with providers to develop aftercare plans.
• Travel within assigned territory to meet enrollees.
• Document outreach and outcomes for program metrics.
