Job summary

UnitedHealth Group is seeking a Behavioral Health Care Advocate to perform case management and care coordination for members on SED and Autism Waivers. You will assess patients, determine levels of care, and manage treatment plans across inpatient and outpatient settings.

Qualified candidates hold an active license in psychology, social work, counseling, or nursing, and have 2+ years post‑licensure. A remote-friendly role with potential travel within the service area is available.

Qualifications

• Licensed master's degree in psychology, social work, counseling, or MFC; PhD or RN with behavioral health experience is preferred.
• Active, unrestricted license in applicable state (e.g., LP/LPC/LCPC/LMSW/LSCSW/LMFT/LCMFT or RN).
• 2+ years post-license experience in a related mental health environment.
• Proficient in MS Word, Excel, Outlook and Teams.

Responsibilities

• Make patient assessments and determine appropriate levels of care.
• Obtain information from providers on outpatient requests for treatment.
• Determine if additional clinical treatment sessions are needed.
• Manage inpatient and outpatient mental health cases throughout treatment plans.
• Administer benefits and review treatment plans.
• Coordinate benefits and transitions between care areas.
• Develop and monitor Person-Centered Service Plans.
• Collaborate with Community Mental Health Centers to ensure services.
