Qualifications

Responsibilities

• Triage incoming calls by evaluating patients' health status per protocols
• Record and report patient conditions and responses to treatments to the care team
• Provide patient and family education on medications and treatments
• Coordinate appointments, tests, and admissions as needed
• Refill medications per standing orders and communicate with pharmacies
• Maintain patient records and ensure timely triage disposition and follow-up scheduling
