Job summary

Trella Health is seeking a Product Manager to own a data domain across post-acute and value-based care. You will turn clinical and claims data into insights to improve transitions of care, reduce readmissions, and deliver better outcomes.

Collaborate with data science, engineering, and GTM teams to ship AI-powered analytics for post-acute providers and value-based organizations. Remote role with a strong focus on impact, data-driven decision making, and healthcare outcomes.

Qualifications

• 4+ years of product management experience in B2B SaaS with direct ownership of roadmap and outcomes.
• Hands-on experience with clinical data sets across care settings (e.g., MDS, OASIS, ADT feeds, claims, EHR data).
• Strong understanding of transitions of care and data/workflows supporting handoffs.
• Experience with value-based care models (e.g., ACOs, bundled payments).
• Experience building data products and analytics/visualization tools.

Responsibilities

• Own product strategy, roadmap, and execution for a clinical data domain across the full care continuum.
• Lead end-to-end product development across cross-functional teams including data science and engineering.
• Translate clinical/claims data into actionable analytics (transitions of care, outcomes, benchmarking).
• Identify and integrate AI/ML capabilities to enhance insights and decision-making.
• Build and ship AI-powered product features from concept to adoption.
