Qualifications

• Must hold an unrestricted Pharmacist license in the state of practice and a Pharmacy degree from an accredited college.
• Ability to perform clinical reviews, assess medications, and collaborate with care teams.
• Knowledge of pharmacy policies, regulatory requirements, and safety practices.

Responsibilities

• Process and evaluate medication orders for completeness and accuracy with patient data.
• Provide clinical recommendations to ensure safe, effective therapy and cost containment.
• Document patient care activities in electronic records and communicate with providers.
• Supervise pharmacy techs and interns; coordinate day-to-day workflow.
• Participate in antibiotic stewardship, pain management, and formulary initiatives.
• Utilize tele-pharmacy technologies for remote order verification across facilities.
