Elite Home Health & Hospice in Lewiston, Idaho, is hiring a full-time Home Health Physical Therapist to join our high-performing team. You will perform in-home PT evaluations, develop mobility-focused plans, provide gait training and balance work, and collaborate with an interdisciplinary team.

Qualifikationen

• Current Idaho Physical Therapist license (or ability to obtain).
• One year of PT experience preferred; home-health experience a plus.
• Strong communication, assessment, and clinical reasoning skills.
• Ability to work independently in a field-based environment.
• Reliable transportation and valid driver's license.

Aufgaben

• Perform comprehensive home health PT evaluations.
• Develop individualized treatment plans focused on mobility, safety, and independence.
• Provide gait training, strengthening, balance training, and home exercise program development.
• Conduct home-safety assessments and recommend DME or environmental modifications.
• Manage a field-based caseload with strong organizational efficiency.
• Collaborate with interdisciplinary team members for coordinated, patient-centered care.
• Maintain accurate, timely documentation in accordance with regulatory and organizational standards.
• Support safe transitions from hospitals, SNFs, and outpatient settings.
