Job summary

Qualifications

• Licensed in the state(s) of practice or eligible for reciprocity.
• Immunization and non-vaccine injectable certification may be required based on area of practice.
• Credentialing and privileging required based on area of practice.

Responsibilities

• Oversees pharmaceutical care, including retrieving and evaluating biomedical literature and designing medication therapy regimens.
• Counsels patients and caregivers on medication therapy, adverse effects, compliance, and appropriate use.
• Reviews and interprets medication orders and prescriptions accurately.
• Participates in management of medical emergencies when applicable.
• Supports medication safety and quality improvement activities; reports events through proper channels.
• Performs quality assurance and improvement of the medication-use system.
• Collaborates with the healthcare team to manage formulary adherence and medication-use process.
• Assists in acquisition of non-formulary, compassionate use, and investigational medications.
• Ensures compliance with organizational policies and state/federal regulations.
• Prepares, dispenses, and oversees proper storage/handling of medications.
• Serves as preceptor for students/residents; mentors pharmacy teammates.
• Directs and checks accuracy of work of pharmacy support staff.
• Demonstrates age-appropriate patient care and adaptability across life span.
