Pay Range: $40.00 - $56.00 per hour

Job Duties
  • Establish and execute skilled occupational therapy treatment plans for residents, including physical agent modalities, therapeutic exercise, therapeutic activities, ADL training, positioning, and use of orthopedic and adaptive equipment.
  • Supervise and collaborate with therapy assistants, passing residents for treatment, and overseeing ongoing care.
  • Complete required documentation in accordance with departmental, agency, and state policies, including Medicare/Medicaid reimbursement documentation.
  • Ensure assessments are completed within established timeframes and that cost information is forwarded to appropriate sources promptly.
  • Provide training for OT‑related services implemented by non‑OT staff as assigned; deliver in‑service training on departmental goals; attend and complete mandatory training.
  • Review medical records, collect data relevant to assessment, conduct interviews with clients, staff, and families; complete initial evaluations and re‑certifications as needed.
  • Attend department meetings, may conduct meetings as assigned, and complete ancillary documentation and communication.
  • Follow all safety rules and regulations, initiating and recommending corrective actions when violations are identified; maintain clean and safe environments and ensure fall‑prevention devices/care plans are followed.
  • Recommend purchase of new equipment; maintain OT office space and equipment in a safe and clean manner.
  • Serve on committees as designated by supervisor and complete special projects as instructed.
  • Perform any other duties assigned by the Director of Rehabilitation.
Minimum Qualifications

  • Ability to lead and coordinate work within the rehabilitation department and across cross‑functional departments.
Requirements
  • Citizenship or legal status: Citizen or national of the U.S., a lawful permanent resident, or an alien authorized to work; the Commonwealth does not provide sponsorship.
  • Exclusion of current DVS employees; successful completion of criminal background check, pre‑employment drug screening, and two TB skin tests.
