Job summary
EMPACT-SPC/La Frontera Arizona is seeking a Rehabilitation Specialist to join the clinical team in Phoenix, AZ. The role involves completing assessments, implementing ISP-based services, and monitoring progress for consumers with serious mental illness.
You will coordinate rehabilitation-related services with RSA counselors, community providers, and others to help clients engage in meaningful community activities or employment. Strong attention to detail and reliable transportation are essential.
Qualifications
- Must have a minimum of High school diploma/GED and 4 years of full time Behavioral Health work experience or meet BHT requirements.
- Two years of work experience in behavioral health, preferably providing vocational/employment and/or rehabilitation services to adults diagnosed with a serious mental illness in an interdisciplinary clinical team environment.
Responsibilities
- Complete assessment and service plan and implement services to consumers in accordance with their ISP.
- Communicate and document consumers’ progress toward their recovery.
- Coordinate and monitor rehabilitation-related services for consumers living with a serious mental illness.
