Job summary
EMPACT-SPC, part of the La Frontera family, seeks a Rehabilitation Specialist in Phoenix, AZ. Qualifications
- High school diploma or GED required; 4 years of full-time behavioral health experience preferred.
- Two years of behavioral health experience in vocational/employment/rehabilitation settings preferred.
- Experience with community agencies/providers in employment or VR is helpful.
- Understanding of best practices in psychiatric rehabilitation is required.
- Basic computer/typing skills are required; familiarity with Outlook/Excel/Word/Next Gen preferred.
Responsibilities
- Complete assessment and service plan per ISP and implement services.
- Document and communicate consumers' progress toward recovery.
- Coordinate and monitor rehabilitation services with RSA VR counselors and providers.
