Job summary

Southwest Network is seeking a Rehabilitation Specialist to support members in increasing employment and independent living skills. You will partner with a vocational rehabilitation counselor, community program providers, and others to implement services in individual service plans.

You will work closely with the clinical team to ensure services are delivered, with emphasis on helping adults with serious mental illness achieve better outcomes through integrated supports and ongoing training.

Qualifications

• Must possess one of the following degrees in behavioral health or related field; references the list of credentials above.
• Minimum one year experience with seriously mentally ill (SMI) adults.
• Experience with vocational, rehabilitation, educational, and social services for SMI adults.
• Must maintain CPR, First Aid, and Therapeutic Options certifications.
• Must have a valid Arizona driver's license and car insurance.
• Must be at least 21 years old.

Responsibilities

• Provide support to members to increase employment and independent living skills.
• Collaborate with vocational rehabilitation counselors, community program providers, and other stakeholders to implement services.
• Work with the clinical team to ensure services in members' individual service plans are carried out.

Education

Master's or bachelor's degree with a behavioral health or related field

Bachelor's degree in a non-related field plus one year related experience

Associate's degree in any field plus one year related experience

High school diploma or equivalency certificate plus three years' related experience
