General Summary

The Rehabilitation Specialist will educate members to function within their community. Treatment can address relationships with family, friends, and co-workers and often helps members work toward living independently. This position will work closely with a team of professionals both within and outside the organization to achieve successful social integration for the member.

Job Responsibilities
  • With the assistance of an integrated care team, develop treatment plans that help the member learn skill sets needed for independent living, monitor patients' progress, help them achieve their social integration goals, and intervene in crisis situations.
  • Rehabilitation Specialists may provide assistance with job training, housing needs, and family support issues.
  • Participate in the assessment, evaluation, and planning of patient care; revise as necessary.
  • Provide community and home-based services to enhance and achieve treatment goals.
  • Participate in Interdisciplinary Team meetings, clinical staffing meetings, case conferences and other meetings as appropriate. Consult with treatment staff to coordinate effectiveness of services contained in the member’s individual service plans and make adjustments where needed.
  • Maintain appropriate documentation (charting) and document accurately and timely in accordance with agency and funding source requirements.
  • Provide member advocacy and education as needed to ensure delivery of required services.
  • Preserve the human dignity and the confidentiality of each member.
  • Follow any policies, procedures, and controls established by the organization, the HIPAA Privacy Officer, and/or the HIPAA Security Officer regarding access to, protection of, and the use of the PHI.
  • Perform other duties as assigned or necessary as they relate to the nature of the position.
Qualifications
  • Education – Bachelor’s degree in a Behavioral Health related field or equivalent experience.
  • Experience – One (1) year of Behavioral Health experience. Experience in a medical or behavioral health clinic setting.
Regulatory
  • Minimum 18 years of age.
  • DPS Level I fingerprint clearance (must possess upon hire and maintain throughout employment).
  • CPR, First Aid, AED certification, if required (must possess upon hire and maintain throughout employment).
  • Current, valid Arizona Driver’s License and 39-month Motor Vehicle Report and proof of vehicle registration liability insurance to meet insurance requirements, if required.
