Job summary

Qualifications

• Bachelor’s/Associate/Master’s degree in an allied health field or comparable skilled nursing experience.
• Current state licensure and/or certification/registry as required.

Responsibilities

• Manage rehabilitation services and maintain a clinical caseload.
• Recruit and retain rehabilitation service personnel.
• Provide onboarding and on-the-job training for staff.
• Foster open communication and problem solving among employees.
• Conduct performance reviews for rehabilitation personnel.
• Oversee rehabilitation services operations at the facility level.
• Coordinate billing reconciliation with finance to ensure accuracy.
• Ensure program standards meet MDH SNF and other regulatory requirements.
• Identify community program development needs and implement appropriate programs.
• Develop clinical affiliation programs with educational institutions.
• Investigate and resolve staff, service, and resident-related issues.
• Review quality and appropriateness of delivered services.
• Attend facility meetings as requested.
• Assess community resources and supplies.
• Track MDS data and communicate results to relevant personnel.
• Coordinate MDS data changes per community procedure.
• Utilize tracking systems and implement needed changes.
• Ensure daily data entry for resource utilization and CPT coding; monitor minutes.
• Assist with quality assurance programs as required.
• Maintain confidentiality standards for residents and the organization.
• Maintain professional relationships with coworkers, residents, families, and community personnel.
• Present a positive, professional image at all times.
• Generate special reports/data to aid leadership.
• Lead in assessing efficiency and cost-effectiveness of rehabilitative services.
• Report to work on time and coordinate schedules for optimal outcomes.
• Maintain a safe, clean work area and comply with infection control and OSHA standards.
• Perform other duties as assigned; pursue continuing education and professional development.
• Adhere to licensure-specific job responsibilities (e.g., Physical Therapist).
