Job summary

Kalesta Healthcare Group seeks a rehabilitation professional to oversee patient care, evaluations, and treatment within a skilled nursing environment. The role involves supervising assistants, coordinating with therapists and physicians, and ensuring compliance with licensure and documentation standards.

You will educate staff, document progress, and participate in program development to enhance patient outcomes in a dynamic, interdisciplinary setting.

Qualifications

• Must maintain appropriate state licensure, certification and credentialing.
• Experience with evaluating patients and developing treatment plans.
• Strong documentation and communication skills with both clinical staff and third-party payers.

Responsibilities

• Evaluations are initiated per MD orders, payer authorization, and facility guidelines.
• Develops treatment plans and provides treatment to achieve the best functional outcome.
• Educates staff and other healthcare professionals on benefits and outcomes of intervention.
• Completes documentation including evaluations, progress notes, summaries, or monthly reports.
• Keys information into computer systems, including EMR and rehab systems.
• Screens individuals who may benefit from intervention and uses professional judgment to ensure safety.
• Contributes to program development and protocol improvements.
