Job summary

Brooks Rehabilitation seeks a Nurse or Therapist Liaison to evaluate referrals and coordinate rehabilitation services for inpatients. The role supports interdisciplinary programs and promotes Brooks services to referral sources while developing relationships with providers.

Responsibilities include treatment planning, admission assessments, staff education, and marketing activities; ongoing collaboration with physicians, social workers, and care teams is required.

Qualifications

• Licensed Nurse (RN, LPN, BSN, MSN) or therapist (PT, OT, COTA, PTA) licenses required.
• Confidentiality and professional ethics are mandatory.
• Active continuing education and credentials in good standing.

Responsibilities

• Evaluates referrals and coordinates recommendations and services for rehabilitation needs.
• Plan, develop, organize, and coordinate interdisciplinary inpatient rehab programs.
• Promote Brooks Rehabilitation services to referral sources and maintain relationships.
• Coordinate pre-admission screening and treatment planning with patients and families.
• Assist with staff education, committees, and external marketing activities.
