Job summary

Hackensack Meridian Health is seeking a licensed Physical Therapist in New Jersey to evaluate and plan goal‑oriented therapy, restore function and prevent disability after trauma or illness. The role emphasizes patient-centered care, accurate documentation, and collaboration with care teams.

The successful candidate will participate in discharge planning, educate residents and families, and implement individualized treatment plans within a supportive, safety‑driven environment.

Qualifications

• Graduate of an APTA accredited program.
• Licensed Physical Therapist.
• Ability to perform patient assessments and develop treatment plans.
• Proficient in clinical documentation and data entry.

Responsibilities

• Committee representation on requests from Supervisor.
• Deliver the highest level of service to residents, families and visitors.
• Discharge planning and updating during treatment course.
• Documentation of progress, re-evaluations and discharge summaries in a timely manner.
• Educate residents and families and ensure safe use of assistive devices.
• Develop and implement individualized Plan of Care and maintain safety standards.
