Hackensack Meridian Health is seeking a licensed Physical Therapist in New Jersey to evaluate, plan, and implement goal-oriented therapy aimed at restoring function and preventing disability after trauma or illness. This role focuses on delivering high-quality care, coordinating discharge plans, and maintaining accurate documentation.

Qualifications

• Graduate of an APTA accredited program or equivalent.
• Physical Therapist License is required.
• Proficient computer skills including MS Office/Google Suite.

Responsabilités

• Delivers the highest level of service to residents, families and visitors.
• Discharge planning established at initial evaluation and updated throughout treatment.
• Documents Progress Notes, Reassessments and Discharge Summaries in a timely manner.
• Maintains safe environment and proper body mechanics during activities.
• Completes resident treatments and scenic plan of care updates as needed.
• Demonstrates appropriate judgment in ordering and fitting prosthetic/orthotic devices.
