Job summary

Qualifications

• High School Diploma or GED required.
• Less than 1 year patient care or customer service experience.
• Ability to recognize and respond to changes in patient condition.
• Willingness to work a flexible schedule to meet patient needs.
• Basic clinical knowledge of rehab admission criteria.
• Competence in monitoring vital signs including HR, BP, RR.
• BLS certification required on hire.

Responsibilities

• Provide care-related activities for designated patients under supervision of RN, PT, OT, ST, EP, or recreational therapist.
• Report observations/changes in patient condition to nursing/medical staff; perform hourly rounding for pain, toileting, and positioning needs.
• Assist with transfer, gait, daily living and transport to therapy as needed.
• Assist patients with dysphasia and ensure dining adherence.
• Complete routine assignments for a patient group within scheduled time.
