Capital Post-Acute in Sacramento, CA is seeking a Director of Rehabilitation to lead the therapy team in a busy skilled nursing environment.

This full-time role offers a competitive salary ($130k-$150k), a $4,000 hiring & retention bonus, and opportunities to mentor therapists while ensuring PDPM compliance and high-quality resident care.

You will collaborate with nursing and physicians, develop care plans, and drive clinical excellence.

Qualifications

• Licensed in CA in Physical Therapy, Occupational Therapy, or Speech-Language Pathology in good standing.
• Minimum of 1 year of experience as a Director of Rehabilitation in a skilled nursing facility is required.
• Bachelor’s degree in Physical Therapy, Occupational Therapy, or Speech-Language Pathology (or higher).
• Prior leadership experience within a rehabilitation department preferred.
• Strong knowledge of PDPM, Medicare, and skilled nursing facility regulations.
• Experience with EMR systems and regulatory compliance.
• Excellent leadership, communication, and organizational skills.
• Ability to foster teamwork, professional growth, and clinical excellence.
• Passion for serving the geriatric population.

Responsabilités

• Lead and manage the daily operations of the rehabilitation department.
• Supervise, mentor, and support Physical Therapists, Occupational Therapists, Speech-Language Pathologists, assistants, and therapy aides.
• Ensure compliance with federal, state, and facility regulations, including documentation standards.
• Collaborate with nursing, physicians, and interdisciplinary teams to coordinate resident care.
• Develop and oversee individualized care plans and support PDPM processes.
• Monitor productivity, clinical outcomes, and departmental performance metrics.
• Participate in audits, care conferences, and leadership meetings.
• Promote a positive, collaborative, and professional work environment.
