Job summary

PRN Home Health & Therapy is seeking a Rehabilitation Coordinator / Certified Occupational Therapy Assistant (COTA) to lead therapy operations in a skilled nursing facility in Fond du Lac area. You will mentor clinicians, coordinate interdisciplinary care, and partner with PRN leadership to drive clinical excellence and resident outcomes.

Ideal candidate holds Wisconsin COTA license, CPR/BLS, SNF experience, and thrives in a leadership role focusing on scheduling, staff development, and program

Qualifications

• Current Wisconsin licensure as a Certified Occupational Therapy Assistant (COTA).
• Current American Heart Association CPR/BLS Certification.
• Experience in a Skilled Nursing Facility (SNF).
• Leadership or rehabilitation coordination experience preferred.
• Strong organizational and time-management skills.
• Excellent interpersonal and communication abilities.
• Ability to mentor others and drive clinical excellence.

Responsibilities

• Serve as the primary therapy liaison between the facility, interdisciplinary team, and PRN leadership.
• Foster relationships with facility partners, residents, families, and healthcare providers.
• Coordinate rehabilitation services to ensure high-quality patient care and outcomes.
• Lead and support a dynamic team of therapists and therapy assistants across disciplines.
• Assist with scheduling and staffing to deliver therapy services efficiently.
• Mentor clinicians by providing guidance and resources for growth and success.
• Participate in care conferences and facility leadership meetings.
• Support onboarding and training of new therapy staff.
• Monitor operations and identify opportunities for program development and service enhancements.
• Collaborate with facility leadership to develop innovative rehab programs.

As a Rehabilitation Coordinator, you will serve as the key connection between facility staff, therapy clinicians, and PRN leadership while continuing to support high-quality rehabilitation services.

What You'll Do

As our Rehabilitation Coordinator, you will:

• Serve as the primary therapy liaison between the facility, interdisciplinary team, and PRN leadership.
• Foster strong relationships with facility partners, residents, families, and healthcare providers.
• Coordinate rehabilitation services to ensure exceptional patient care and positive clinical outcomes.
• Lead and support a dynamic team of therapists and therapy assistants across multiple disciplines.
• Assist with scheduling and staffing to ensure therapy services are delivered efficiently and effectively.
• Mentor clinicians by providing guidance, support, and resources to promote professional growth and success.
• Participate in care conferences, interdisciplinary meetings, and facility leadership meetings.
• Support onboarding, orientation, and training of new therapy team members.
• Monitor therapy operations and help identify opportunities for program development and service enhancement.
• Collaborate with facility leadership to develop innovative rehabilitation programs that improve resident outcomes and quality of life.
• Promote a positive work culture focused on teamwork, communication, accountability, and excellence.
• Ensure therapy services remain compliant with company policies, facility requirements, and regulatory standards.

Qualifications:
