Job summary

Horizon House Inc. is seeking a Full Time Rehabilitation Counselor to join the Developmental Services Division in East Norriton, PA. The role focuses on supporting residents with behavioral health and developmental needs in a community-based setting.

You will provide care, promote growth toward independence, and help document progress within individualized service plans. A.A. degree and crisis intervention skills are required, along with a valid driver’s license.

Qualifications

• A.A. degree in the social sciences or health care related field with one year of relevant MH/MR experience or equivalent.
• Crisis intervention/prevention and de-escalation skills are required.
• Good oral and written communication skills are essential.
• Ability to work independently and as part of a team.
• Must possess a valid driver's license and an acceptable driving record.

Responsibilities

• Provide care and supervision of residents to ensure a safe, healthy, secure environment.
• Facilitate client growth toward independence in self-care, daily living, socialization and community living.
• Participate in developing individual service plans and document client progress throughout the service delivery system.
