Job description

Beautiful Hearts and Minds Health Services is currently looking for full time/part time contractual rehabilitation coordinators. Rehabilitation Coordinators provide direct support, education and outreach to a caseload of adults and/or minors recovering from severe mental illness. The primary focus is meeting with individuals within their natural environments, coordinating community support systems and services, crisis intervention, education and assisting in removing barriers to improving community integration and functioning.

Rehabilitation Coordinators will be required to:

complete rehab assessments

complete Initial and concurrent treatment plans

complete treatment plan reviews

provide and coordinate community resources

meet participants in their community, home, job

facilitate and coordinate group activities.

Candidates must have minimum high school diploma and at least 1 year PRP experience. Candidates must be have excellent written and oral communication. This is a hybrid work environment (work is split between office, home and in the community).
