Qualifications

• RN licensure in Louisiana; may have fifth year college program certificate or 2–4 years related experience
• Knowledge of basic bookkeeping, office procedures, and electronic billing procedures
• BCLS certification required

Responsibilities

• Meets with patient on admission to discuss rehabilitation process and the case manager's role.
• Conducts and documents initial patient assessment within 72 hours with psychosocial and disposition status; helps set discharge goals.
• Coordinates, documents and reviews interdisciplinary team goals tied to care plan and discharge planning.
• Maintains up-to-date documentation in medical records; communicates with payers and services involved in patient care.
• Serves as primary family contact for the treatment team and coordinates family conferences and post-discharge follow-up.
