Job summary

Virginia Mason Rehabilitation is seeking a PRN Case Manager to collaborate with a multidisciplinary team, coordinate care, and document per CMS guidelines. The role emphasizes annual competencies, IPoC documentation, and effective family/caregiver communication in a facility within Lifepoint Health.

The position requires a nursing or social work degree, BLS within 30 days, ACLS/PALS within six months, and 1 year of experience preferred, with comprehensive benefits and growth opportunities

Qualifications

• Bachelor's degree in Nursing or Social Work from an accredited program.
• BLS certification required within 30 days of hire.
• ACLS and PALS certifications required within six months of hire.
• One year of experience preferred.
• ASLS certification preferred.

Responsibilities

• Complete departmental orientation and initial/annual competencies.
• Collaborate with interdisciplinary staff to identify barriers to care or discharge.
• Document IPoC per CMS guidelines within timelines.
• Schedule and conduct family conferences and caregiver communications.
• Facilitate weekly care team conferences to review progress and adjust treatment plans.
• Review CMG and document treated comorbid conditions; notify HIM as needed.
• Perform other duties as assigned.
