Job summary

Encompass Health Corporation is seeking a Case Manager to coordinate patient care from admission through post-discharge, working with an interdisciplinary team to ensure effective treatment plans and timely discharges.

The role emphasizes clinical expertise, regulatory knowledge, and patient-centered care in a nationwide inpatient rehabilitation setting. This opportunity offers growth within a leading rehabilitation provider and a supportive team environment.

Qualifications

• Must be qualified to independently complete an assessment within the scope of practice of the discipline (e.g., RN, SW, OT, PT, ST, Rehab Counseling).
• Active license required if licensure is mandated for the discipline in the state.
• CCM® or ACM™ certification required OR must be obtained within two years of placement in Case Manager II.

Responsibilities

• Collaborate with interdisciplinary team to guide treatment plans based on patient needs.
• Coordinate tentative discharge plan and contingency plans with the team.
• Participate in planning and execution of discharge experience for patients.
• Monitor patient experience, including quality, timeliness, service appropriateness, and payor factors.
• Lead weekly team conferences and coordinate treatment plan modifications as needed.
• Complete case management addendums and required documentation.
