Job summary

Learning Services, a Collage Rehabilitation Partner, is seeking a Case Manager for our Gilroy, CA program to support adults in neurorehabilitation.

You will oversee active rehab caseloads, coordinate with the treatment team, and ensure treatment plans from admission to discharge, with travel up to 30% as needed.

A master’s degree or licensed clinician (RN) is required, along with case management experience in healthcare rehabilitation.

• Must have a valid driver’s license and maintain responsible driving behaviors.

Responsibilities

• Oversee all case provisions for the active rehab caseload in residential neurorehabilitation and day treatment.
• Lead team meetings and coordinate rehabilitation goals with the treatment team and discharge plan.
• Ensure implementation of treatment plans from admission through discharge and follow-up.
• Collaborate with physicians and other health care providers outside the primary team.
• Coordinate schedules, therapies and other appointments/services.
• Maintain regular contact with families and funders; document clinical notes and reports.
• Assist the Program Director with onboarding and training of case managers.

We are searching for a Case Manager for our program located in Gilroy, CA.

Essential Duties and Functions:

• Responsible for all case provisions for the active rehabilitation caseload, for both the residential neurorehabilitation and day treatment clients.
• Leads team meetings and interfaces with the treatment team to establish rehabilitation goals and coordinates treatment necessary to support the client’s treatment plan. Assists the team on goals and the discharge plan.
• Responsible for ensuring the implementation of the clients’ treatment plan(s), from admission through discharge and follow-up
• In consultation with the clinical team, identifies, collaborates, and consults with physicians and other health care providers outside of the primary treatment team.
• Coordinates schedules, therapies, and other appointments/services
• Responsible for regular contact with families and funders
• Ongoing internal report writing, clinical documentation, and chart documentation
• Facilitating funding issues as needed.
• Serves as client advocate by communicating clients’ needs and issues to team members and other designated individuals, including a plan for resolution and follow-through as indicated.
• Support the Program Director in program case managers’, onboard training, and performance to ensure case management standards of performance meet expectations.

Requirements:

• A master’s degree or a licensed clinician, including an RN, is required.
• Case management in the healthcare industry/rehabilitation experience is required
• Must have a valid driver's license and maintain responsible driving behaviors

Physical Requirements & Working Conditions:

• Regularly required to communicate, both orally and in writing; occasionally required to speak on the telephone for extended periods of time.
• Regularly required to demonstrate a high degree of attention to detail, quality standards, and accuracy. Must have the ability to handle and organize multiple priorities and deadlines.
• Must have the ability to travel up to 30% of the time spent working in this position.
