The role requires a valid RN license and case management experience in healthcare/rehabilitation, plus a valid driver's license. Travel up to 30% may be required as you liaise with families, funders, and treatment teams to ensure high-quality, ethical patient care.

Responsibilities

• Responsible for all case provisions for the active rehabilitation caseload, for residential and day treatment clients.

• Collaborates with physicians and other health care providers outside the primary team.
• Coordinates schedules, therapies, and other appointments/services.
• Maintains regular contact with families and funders.
• Produces internal reports and clinical documentation.
• Facilitates funding issues as needed.
• Advocates for clients by communicating needs and arranging resolutions.
• Supports Program Director in onboarding training and performance to meet standards.

We are searching for an RN Case Manager for our program located in Gilroy, CA.

Essential Duties and Functions:

• Responsible for all case provisions for the active rehabilitation caseload, for both the residential neurorehabilitation and day treatment clients.
• Leads team meetings and interfaces with the treatment team to establish rehabilitation goals and coordinates treatment necessary to support the client's treatment plan. Assists the team on goals and the discharge plan.
• Responsible for ensuring the implementation of the clients' treatment plan(s), from admission through discharge and follow-up
• In consultation with the clinical team, identifies, collaborates, and consults with physicians and other health care providers outside of the primary treatment team.
• Coordinates schedules, therapies, and other appointments/services
• Responsible for regular contact with families and funders
• Ongoing internal report writing, clinical documentation, and chart documentation
• Facilitating funding issues as needed.
• Serves as client advocate by communicating clients' needs and issues to team members and other designated individuals, including a plan for resolution and follow-through as indicated.
• Support the Program Director in program case managers', onboard training, and performance to ensure case management standards of performance meet expectations.

Requirements:

• Valid Registered Nurse License
• Case management in the healthcare industry/rehabilitation experience is required
• Must have a valid driver's license and maintain responsible driving behaviors

Physical Requirements & Working Conditions:

• Regularly required to communicate, both orally and in writing; occasionally required to speak on the telephone for extended periods of time.
• Regularly required to demonstrate a high degree of attention to detail, quality standards, and accuracy. Must have the ability to handle and organize multiple priorities and deadlines.
• Must have the ability to travel up to 30% of the time spent working in this position.
