Job summary

Placements24 is seeking a Rehabilitation Case Manager to join our team in Welkom. This hybrid role combines in-person assessments with remote administration and coordination to help clients reintegrate into the community.

You will assess needs, develop care plans, link to therapy and medical services, and monitor progress while collaborating with families and professionals to ensure holistic support.

Qualifications

• Relevant qualification in Social Work, Occupational Therapy, Psychology, or related field.
• Experience in case management within rehabilitation or disability services preferred.
• Strong communication and problem-solving abilities.

Responsibilities

About the Role

Our client is seeking an experienced and compassionate Rehabilitation Case Manager to join their team in Welkom . This hybrid role involves working closely with individuals undergoing rehabilitation, assisting them in their recovery journey towards independence and reintegration into the community. You will assess client needs, coordinate support services, and monitor progress, utilizing a blend of in-person client interactions and remote administrative and communication tasks. This position plays a vital role in supporting individuals in the Free State province as they overcome challenges and rebuild their lives.

Key Responsibilities

• Conduct comprehensive assessments of clients' physical, emotional, and social rehabilitation needs.
• Develop individualized rehabilitation plans in collaboration with clients and healthcare professionals.
• Coordinate and facilitate access to a range of support services, including therapy, medical care, and vocational training.
• Monitor client progress, provide ongoing support, and adjust care plans as necessary.
• Liaise with family members, caregivers, and other relevant stakeholders to ensure holistic support.
• Maintain accurate and up-to-date case notes and progress reports in the client management system.

Requirements

• Relevant qualification in Social Work, Occupational Therapy, Psychology, or a related field.
• Demonstrated experience in case management, preferably within rehabilitation or disability services.
• Strong understanding of rehabilitation principles and available support services.
• Excellent communication, empathetic listening, and problem-solving skills.
• Ability to manage a caseload effectively and work autonomously, as well as part of a team.
• Proficiency in relevant software and a commitment to continuous professional development.
