Job summary

Advocate Health is seeking a Registered Nurse for the Rehabilitation Unit in Charlotte, NC. The role involves planning, coordinating, and monitoring care, with emphasis on rehabilitation for critically and chronically ill patients.

The position is full-time, 40 hours per week, day shift, with potential weekends/holidays as needed. Qualifications include a Bachelor’s in Nursing, current RN license in the state, and BLS.

Qualifications

• Bachelor of Nursing required; Master’s preferred.
• RN license in the applicable state required; CRRN preferred.
• 3–5 years of related rehabilitation clinical experience highly recommended.
• BLS required by policy.

Responsibilities

• Demonstrates the process of planning, organizing, coordinating, and monitoring the services and resources needed to respond to an individual's health care needs.
• Provides ongoing evaluation of the individual's progress on the plan and effectiveness of services across hospitalization.
• Represents the patient in care team and advocates for effective services to ensure quality of care and attainment of goals.
• Integrates knowledge of disease and medical treatments in holistic assessment and differential diagnosis.
• Identifies functional alterations and formulates nursing diagnoses.
• Assesses potential challenges in physiological/psychosocial function.
• Assists patient and family in planning for re-entry to home or alternate living site.
• Designs, implements, and evaluates population-based care programs to enhance outcomes.
• Leads education for nursing staff and patients/families in area of specialty.

Education

Bachelor's Degree in Nursing

Master's in Nursing preferred

RN license in state required

CRRN preferred
