Total Extended Care Services in Illinois is seeking a Nurse Practitioner or Physician Assistant to provide on-site rehabilitation-focused care in skilled nursing facilities, partnering with a PM&R physician and therapy team. You will conduct assessments, coordinate with therapists and primary care providers, and document visits.

The role offers a flexible schedule, no on-call evenings or weekends, and the possibility to complete documentation from home while maintaining strong clinical presence

Competenze

• Licensed and certified Nurse Practitioner or Physician Assistant with active licensure in Illinois.
• Experience in skilled nursing or senior living environments.
• Strong communication and collaboration skills with clinicians, therapists, and staff.
• Detail-oriented, reliable and organized with flexible, outcomes-focused scheduling.

Mansioni

• Provide on-site rehabilitation-focused care in skilled nursing facilities under the guidance of a PM&R physician.
• Conduct thorough assessments and follow-up visits to optimize function and pain control for sub-acute and long-term residents.
• Partner with therapists to align treatment plans and ensure continuity of rehab care.
• Coordinate care and communicate with primary care providers, nursing staff, social workers and families for safe transitions.
• Complete accurate, timely charting and documentation, with flexibility to perform some documentation from home.
