Job summary

Total Extended Care Services in Peoria, IL is seeking a Nurse Practitioner or Physician Assistant to join our PM&R-focused team in sub-acute rehab settings.

You will partner with a PM&R physician, collaborate with therapists, nurses, social workers, and families to improve patient outcomes, reduce readmissions, and provide consistent care in skilled nursing facilities. Flexible full-time and part-time options are available; on-site care with some home-based charting.

Qualifications

• Licensed Nurse Practitioner (Required)
• Certified Nurse Practitioner (Required)
• Licensed Physician Assistant (Required)
• Certified Physician Assistant (Required)
• Must be licensed in IL
• Reliable transportation

Responsibilities

• Partner with the facility's therapy department to provide the best possible rehab care.
• Communicate and collaborate with the patient's primary care team to ensure optimal outcomes.
• Timely charting/documentation is required and can be done from home when appropriate.
