Total Extended Care Services is seeking a Nurse Practitioner or Physician Assistant to join our rehabilitation team in Illinois. The role partners with PM&R physicians in sub-acute rehab settings, focusing on improving patient outcomes in skilled nursing facilities.

Responsibilities

• Partner with the facility's therapy department to provide rehab care and coordinate with the patient’s primary care team to ensure the best outcomes.
• Timely charting/documentation is required, with option to complete from home when appropriate.
