Job summary

HCA Houston Healthcare Southeast in Pasadena, TX is seeking an autonomous Clinical Rehab Liaison to act as a clinical consultant and market specialist across a regional network. You will perform pre-admission screenings, evaluate medical necessity under rehab criteria, and build strong referral partnerships with physicians and case managers.

The role accepts experienced RNs, LPTs, ROTs, or SLPs with strong diagnostic acumen in a consultative environment.

Qualifications

• Graduate of an accredited clinical program.
• Licensed professional in Texas in a clinical health care specialty (RN/PT/OT/SLP) is required.
• Current BLS certification is required.
• Minimum 2 years of recent clinical care experience.

Responsibilities

• Act as inpatient rehabilitation expert between internal and external referral sources and establish relationships with case managers and physicians.
• Coordinate intake and pre-admission screening process and document rehab admission criteria.
• Serve as primary contact for case management and referring physicians with timely follow-ups.
• Assist in developing the annual strategic plan for Rehab Unit/Hospital and outreach programs.
