Job summary

Qualifications

• Bachelor's degree preferred.
• Current clinical licensure required (state-specific).
• Prior marketing/sales experience and clinical assessment skills preferred.
• EMR proficiency and basic data-entry ability.
• Excellent oral and written communication; strong interpersonal and presentation skills.

Responsibilities

• Builds and maintains referral relationships to drive admissions to the hospital.
• Coordinates intake and pre-admission screening process and conducts on-site clinical assessments.
• Educates patients and families on rehabilitation options, level of care, and participation requirements.
• Serves as the initial contact for external case management, discharge planners, payors, and hospital partners.
• Conducts face-to-face educational visits with physicians, case managers, and other professionals.
• Educates community decision-makers about hospital and acute rehab services to develop census through outreach.
• Documents screenings, decisions, and communications accurately in EMR and tracking systems.
• Performs other duties as assigned.
