Job summary

Advanced Health Care of Aurora is hiring a licensed Occupational Therapist to assess, plan, and implement rehabilitative programs for residents in our skilled nursing facility.

This PRN position covers Weekdays and Weekends with 8-hour shifts. You will collaborate with interdisciplinary teams, develop treatment plans, document progress, and educate residents and families on techniques and home programs.

Qualifications

• Current state licensure or eligibility for licensure as an Occupational Therapist.
• Clinical experience in a skilled nursing facility or similar setting preferred.
• Ability to build rapport with residents through a compassionate and caring demeanor.
• Excellent interpersonal and communication skills for collaboration with residents, families, and team members.
• Commitment to resident-centered care with empathy and compassion.
• Strong clinical skills in assessment, treatment planning and intervention implementation.

Responsibilities

• Assess residents’ physical, cognitive, and psychosocial functioning.
• Develop individualized treatment plans based on assessment findings and goals.
• Provide OT interventions: ADL training, therapeutic exercises, adaptive equipment, environmental modifications.
• Document resident progress, interventions, and discharge plans per policy.
• Collaborate with physicians, nurses, PTs and SLPs for coordinated care.
• Educate residents and families on techniques, safety measures, and home exercise programs.
• Participate in team meetings and contribute to care planning and goal setting.
• Stay current with best practices, evidence-based interventions, and regulatory changes in OT.
• Maintain licensure and adhere to ethical standards.
• Contribute to a positive work environment with professionalism and teamwork.
• Perform all other duties assigned.
