Job summary

Enhance Rehabilitation in Aurora, IL is seeking qualified Occupational Therapist candidates to join our dynamic team in a SNF setting, evaluating and treating patients following an individualized plan of care.

You will collaborate with facility staff to ensure excellent care, develop plans of care with measurable goals, educate patients and families, and maintain accurate daily progress notes and documentation. Supervising OTAs and ongoing patient communication are essential parts of the role.

Responsibilities

• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, addressing concerns and educating them on injury, progress and outcomes
• Re-evaluating patient progress and revising plans of care to meet goals
• Complete timely documentation including evaluations, re-evaluations, discharges and daily progress notes
• Supervising Occupational Therapy Assistants
• Collaborate with facility staff to ensure excellent patient care
