Qualifications

• Education required: MSW or RN; masters in related field may be considered.
• Two to five years of clinical healthcare experience preferred, rehab setting preferred.
• Full legal name and current home address required; resume upload encouraged.

Responsibilities

• Performs admission review identifying psycho-social barriers, discharge needs and plan of care.
• Completes ongoing discharge planning assessments and mitigations for timely discharge.
• Maintains documentation and facilitates compassionate issue resolution in patient care.
• Collaborates with patient, family and interdisciplinary team to design discharge plans.
• Maintains contact with patient/family multiple times weekly regarding rehab progress and plans.
• Participates in daily stand-up meetings and weekly interdisciplinary team meetings.
• Arranges services prior to discharge to meet patient needs and reduce readmission.

Education

MSW

RN (BSN preferred)

Masters in related field (physical, occupational, or speech therapy)
