Job summary

Legacy Healthcare Services, Inc. is seeking a Rehab Director to lead the therapy team in a senior living facility. You will promote Legacy policies, programs, and patient-centered care, while growing clinical leadership and program development.

You will build relationships with facility staff, manage productivity, and ensure compliant, high-quality therapy services for aging adults across disciplines.

Qualifications

  • Clinical background in PT, PTA, OT, COTA, or SLP.
  • Three to five years of experience in long-term care or senior living.
  • Licensed and registered in the relevant state(s).

Responsibilities

  • Create rapport and positive working relationships with facility personnel and Legacy employees.
  • Oversee internal marketing to build relationships with staff and residents.
  • Manage site productivity to meet budget requirements.
  • Maintain a caseload to support rehab department goals.
  • Assess patient needs and develop facility-appropriate clinical programs.
  • Direct and train staff in clinical service delivery across disciplines.
  • Coordinate daily rehab activities with facility staff to plan therapy programming.
  • Participate in clinical outcomes reporting and share results with customers.
  • Manage staff schedules to meet caseload demands and use scheduling tools.
  • Oversee CliniFax and ensure physician signatures on therapy plans.
  • Foster team communication among rehab staff, facility staff, and Legacy corporate personnel.
  • Assist with staff orientation/training to meet standards.

As a Rehab Director with Legacy Healthcare Services, you serve as the primary Legacy representative at the facility, promoting Legacy policies, clinical programs and supporting our mission to help aging adults live safe, healthy lives full of strength, movement, and joy.   • Create rapport and positive working relationships with key facility personnel and fellow Legacy employees that allows for discovery of others’ needs.
  • Oversee internal marketing efforts to assist with relationship building with staff and residents while keeping community leaders apprised of Legacy’s programs and other offerings.
  • Manage site productivity and makes necessary changes to achieve budget.
  • Maintain a caseload that assists the rehab department in meeting standards.
  • Able to assess patient needs and develop clinical programs appropriate for the facility census.
  • Able to direct and train others in clinical service delivery across disciplines.
  • Daily coordination of rehab department with facility staff to plan and implement therapy programming.
  • Participates in clinical outcomes reporting and shares with the customer.
  • Manages staff schedules to meet the caseload requirements and utilizes the scheduling tools for patient schedules.
  • Manages CliniFax and all requirements of physician signatures on the therapy plan of cares.
  • Creates and maintains team relationships and open communication with and between rehab staff, facility staff, and Legacy corporate personnel.
  • Assists with orientation/training for staff with guidance and assistance from ARD/ MSM to meet company, state and federal standards.

Position requirements:
  • Clinical background as a Physical Therapist (PT), Physical Therapy Assistant (PTA), Occupational Therapist (OT), Certified Occupational Therapy Assistant (COTA), or Speech Language Pathologies (SLP).
  • Three to five years’ experience in long-term care (LTC) or senior living. Some prior supervisory experience preferred but not required.
  • Minimum of a degree from accredited college or university or equivalent experience (i.e., degree in PT, PTA, OT, COTA, SLP).
  • Licensed and registered (PT, PTA, OT, COTA, SLP) in state where assigned.
