Job summary

Legacy Healthcare Services is seeking a Rehab Director in Whitwell, TN to lead the facility’s rehabilitation program, promote Legacy policies, and support aging adults in living safe, healthy lives. This on-site role emphasizes relationship building with staff and residents, program development, and ensuring standards are met.

The position requires supervising PT/PTA/OT/COTA/SLP staff, coordinating therapy services, and reporting outcomes across the facility and corporate teams.

Qualifications

• Three to five years’ experience in long-term care or senior living.
• Licensed and registered in state where assigned.
• Degree from accredited college or university in PT, PTA, OT, COTA, or SLP.

Responsibilities

• Create rapport and positive working relationships with facility personnel to discover needs.
• Oversee internal marketing efforts to build relationships with staff and residents.
• Manage site productivity to meet budget.
• Maintain a caseload to assist rehab department in meeting standards.
• Direct and train others in clinical service delivery across disciplines.
• Coordinate daily rehab department activities with facility staff to plan therapy programming.
• Participate in clinical outcomes reporting and share results with the customer.
• Manage staff schedules to meet caseload requirements and use scheduling tools.

As a Rehab Director with Legacy Healthcare Services, you serve as the primary Legacy representative at the facility, promoting Legacy policies, clinical programs and supporting our mission to help aging adults live safe, healthy lives full of strength, movement, and joy. • Create rapport and positive working relationships with key facility personnel and fellow Legacy employees that allows for discovery of others’ needs.
• Oversee internal marketing efforts to assist with relationship building with staff and residents while keeping community leaders apprised of Legacy’s programs and other offerings.
• Manage site productivity and makes necessary changes to achieve budget.
• Maintain a caseload that assists the rehab department in meeting standards.
• Able to assess patient needs and develop clinical programs appropriate for the facility census.
• Able to direct and train others in clinical service delivery across disciplines.
• Daily coordination of rehab department with facility staff to plan and implement therapy programming.
• Participates in clinical outcomes reporting and shares with the customer.
• Manages staff schedules to meet the caseload requirements and utilizes the scheduling tools for patient schedules.
• Manages CliniFax and all requirements of physician signatures on the therapy plan of cares.
• Creates and maintains team relationships and open communication with and between rehab staff, facility staff, and Legacy corporate personnel.
• Assists with orientation/training for staff with guidance and assistance from ARD/ MSM to meet company, state and federal standards.

• Clinical background as a Physical Therapist (PT), Physical Therapy Assistant (PTA), Occupational Therapist (OT), Certified Occupational Therapy Assistant (COTA), or
  

Speech Language Pathologies (SLP).

• Three to five years’ experience in long-term care (LTC) or senior living. Some prior supervisory experience preferred but not required.
• Minimum of a degree from accredited college or university or equivalent experience (i.e., degree in PT, PTA, OT, COTA, SLP).
• Licensed and registered (PT, PTA, OT, COTA, SLP) in state where assigned.
