Job summary

Greenfield Rehab seeks an experienced Rehabilitation Director (OT or COTA) to lead therapy services in a Wisconsin skilled nursing facility. The role combines clinical leadership with administrative oversight, guiding PT/OT/SLP teams and ensuring high-quality, compliant patient care.

The director will provide direct patient care when needed, foster collaboration across disciplines, promote staff development, and maintain regulatory standards in a mission-driven environment.

Qualifications

• Licensed by the State of Wisconsin to practice as an Occupational Therapist (OT) or Occupational Therapist Assistant (COTA).
• Bachelor's degree (OT) required; master's or doctoral preferred.
• Minimum 1 year of experience as a treating clinician.

Responsibilities

Job Location: Viroqua - Viroqua, WI 54665. Position Type: Full Time. Join our team as a Rehab Director – Occupational Therapist or Occupational Therapist Assistant.

Greenfield Rehab is seeking an experienced and dedicated Occupational Therapist (OT) or Occupational Therapist Assistant (COTA) to serve as our Rehab Director and lead therapy services in our skilled nursing facility in Viroqua, WI.

The Rehabilitation Director is responsible for the overall leadership, coordination, and management of the rehabilitation department within a skilled nursing facility. This role provides clinical and administrative oversight of physical, occupational, and speech therapy services while maintaining a high standard of patient care, regulatory compliance, and operational efficiency. The Rehabilitation Director also functions as a licensed Occupational Therapist or Occupational Therapist Assistant, providing direct patient care as needed.

OT Qualifications

• Graduate from an accredited school with a minimum of a bachelor's degree in
• Graduate from an accredited school with a bachelor's, master’s, or doctoral degree in Occupational Therapy.
• Licensed by the State of Wisconsin to practice as an Occupational Therapist.
• 1 year experience as a treating clinician required

OTA Qualifications

• Graduate from an accredited school with an associate degree in Occupational Therapy Assistant.
• Licensed by the State of Wisconsin to practice as an Occupational Therapy Assistant.
