Greenfield Rehab is seeking an experienced Physical Therapist to serve as Rehab Director at our skilled nursing facility in Oconomowoc, WI. You will lead therapy services, ensure regulatory compliance, and provide direct PT care as needed.

The role emphasizes clinical leadership, team collaboration, and delivering high-quality, patient-centered rehabilitation to promote senior independence and quality of life.

Qualifications

• Licensed by the State of Wisconsin to practice as a Physical Therapist.
• 1 year of experience as a treating clinician required.
• Graduate from an accredited program with a bachelor’s, master’s, or doctoral degree in Physical Therapy.

Responsibilities

• Lead the rehabilitation department within a skilled nursing facility.
• Coordinate and oversee physical, occupational, and speech therapy services; ensure regulatory compliance.
• Provide direct patient care as a licensed Physical Therapist when needed.

This role provides clinical and administrative oversight of physical, occupational, and speech therapy services while maintaining a high standard of patient care, regulatory compliance, and operational efficiency.
