Job summary

Legacy Healthcare Services is seeking a Rehab Director OT, COTA, PT or PTA to lead rehabilitation services at our Florida facility. You will promote Legacy programs, coordinate therapy services with facility staff, and drive patient outcomes while maintaining strong relationships with residents and families.

The role includes supervising staff, developing evidence-based clinical programs, and ensuring compliance with state and federal standards.

Qualifications

• Clinical background as PT/OT/SLP with LTC/senior living experience.
• Three to five years in long-term care; supervisory experience preferred but not required.
• Degree from accredited college/university in PT, PTA, OT, COTA or SLP.
• Licensed and registered in the state where assigned.

Responsibilities

• Promote Legacy policies and rehab programs to patients and staff.
• Oversee internal marketing to build relationships with staff and residents.
• Manage site productivity and adjust to meet budget.
• Maintain a caseload to assist the rehab department in meeting standards.
• Able to assess patient needs and develop clinical programs.
• Able to direct and train others in clinical service delivery across disciplines.
• Coordinate daily rehab department activities with facility staff.
• Participate in clinical outcomes reporting and share results.
• Schedule staff to meet caseload requirements using scheduling tools.
• Manage CliniFax and physician signatures on therapy plans.

As a Rehab Director OT, COTA, PT or PTA with Legacy Healthcare Services, you serve as the primary Legacy representative at the facility, promoting Legacy policies, clinical programs and supporting our mission to help aging adults live safe, healthy lives full of strength, movement, and joy. • Create rapport and positive working relationships with key facility personnel and fellow Legacy employees that allows for discovery of others’ needs.
• Oversee internal marketing efforts to assist with relationship building with staff and residents while keeping community leaders apprised of Legacy’s programs and other offerings.
• Manage site productivity and makes necessary changes to achieve budget.
• Maintain a caseload that assists the rehab department in meeting standards.
• Able to assess patient needs and develop clinical programs appropriate for the facility census.
• Able to direct and train others in clinical service delivery across disciplines.
• Daily coordination of rehab department with facility staff to plan and implement therapy programming.
• Participates in clinical outcomes reporting and shares with the customer.
• Manages staff schedules to meet the caseload requirements and utilizes the scheduling tools for patient schedules.
• Manages CliniFax and all requirements of physician signatures on the therapy plan of cares.
• Creates and maintains team relationships and open communication with and between rehab staff, facility staff, and Legacy corporate personnel.
• Assists with orientation/training for staff with guidance and assistance from ARD/ MSM to meet company, state and federal standards.

Position requirements:

• Clinical background as a Physical Therapist (PT), Physical Therapy Assistant (PTA), Occupational Therapist (OT), Certified Occupational Therapy Assistant (COTA), or
• Speech Language Pathologies (SLP).
• Three to five years’ experience in long-term care (LTC) or senior living. Some prior supervisory experience preferred but not required.
• Minimum of a degree from accredited college or university or equivalent experience (i.e., degree in PT, PTA, OT, COTA, SLP).
• Licensed and registered (PT, PTA, OT, COTA, SLP) in state where assigned.
