Legacy Healthcare Services is seeking a Rehab Director to serve as the primary company representative at the facility, promoting Legacy policies and clinical programs while helping aging adults live safe, healthy lives with strength and joy.

Qualifications

  • Clinical background as PT/PTA/OT/COTA/SLP.
  • Three to five years’ experience in long‑term care (LTC) or senior living.
  • Licensed and registered in the state where assigned.

Responsibilities

  • Create rapport and positive working relationships with facility personnel and Legacy employees.
  • Oversee internal marketing to foster relationships with staff and residents.
  • Manage site productivity and implement budget-driven changes.
  • Maintain a caseload to support rehab department standards.
  • Assess patient needs and develop appropriate clinical programs.
  • Direct and train staff in clinical service delivery across disciplines.
  • Coordinate daily rehab department activities with facility staff to plan and implement therapy programming.
  • Participate in clinical outcomes reporting and share results with stakeholders.
  • Schedule staff to meet caseload requirements and use scheduling tools.
  • Ensure physician signatures on therapy plans (CliniFax) are obtained.
  • Foster open communication between rehab staff, facility staff, and Legacy corporate personnel.
  • Assist with staff orientation/training to meet standards.

  • Create rapport and positive working relationships with key facility personnel and fellow Legacy employees that allows for discovery of others’ needs.
  • Oversee internal marketing efforts to assist with relationship building with staff and residents while keeping community leaders apprised of Legacy’s programs and other offerings.
  • Manage site productivity and makes necessary changes to achieve budget.
  • Maintain a caseload that assists the rehab department in meeting standards.
  • Able to assess patient needs and develop clinical programs appropriate for the facility census.
  • Able to direct and train others in clinical service delivery across disciplines.
  • Daily coordination of rehab department with facility staff to plan and implement therapy programming.
  • Participates in clinical outcomes reporting and shares with the customer.
  • Manages staff schedules to meet the caseload requirements and utilizes the scheduling tools for patient schedules.
  • Manages CliniFax and all requirements of physician signatures on the therapy plan of cares.
  • Creates and maintains team relationships and open communication with and between rehab staff, facility staff, and Legacy corporate personnel.
  • Assists with orientation/training for staff with guidance and assistance from ARD/ MSM to meet company, state and federal standards.

Position requirements:
  • Clinical background as a Physical Therapist (PT), Physical Therapy Assistant (PTA), Occupational Therapist (OT), Certified Occupational Therapy Assistant (COTA), or Speech Language Pathologies (SLP).
  • Three to five years’ experience in long‑term care (LTC) or senior living. Some prior supervisory experience preferred but not required.
  • Minimum of a degree from accredited college or university or equivalent experience (i.e., degree in PT, PTA, OT, COTA, SLP).
  • Licensed and registered (PT, PTA, OT, COTA, SLP) in state where assigned.
