Education:-Required: Completion of an accredited OTA program Experience:-Preferred: Prior experience in long term care setting and service coordination Licenses/Certificates:-Required: Current PTA or OTA license to practice in the state(s) assigned-Required: CPR, as required by community -Required: Valid driver license and car insurance as required by state law. -Required: Other items as required by federal, state and local standards
