Job summary

Responsibilities

• Provides leadership to the health care team with emphasis on patient care coordination and throughput.
• Discharge planning and coordinating patient flow with rounds.
• Act as liaison between patient, family, physician and interdisciplinary team regarding plan of care and progress towards goals.
• Ensure staff performance through evaluation and orientation.
• Assist with staffing needs and safe discharge planning.
