Job summary

Encompass Health is seeking a Case Manager in Bangor, ME to coordinate patient care from admission to post-discharge and oversee interdisciplinary plans of care. The role emphasizes evaluating needs and ensuring timely, effective discharge planning within regulatory guidelines.

You will collaborate with an interdisciplinary team, monitor patient experience, and contribute to quality outcomes while supporting patients and families in a home-like environment at a leading inpatient rehabilitation

Qualifications

• Must be eligible to independently complete an assessment within the scope of practice of one's discipline.
• CCM® or ACM™ certification required or must be obtained within two years of starting the role.
• Minimum of a bachelor's degree; graduate degree preferred for all licensed/certified health professionals.

Responsibilities

• Coordinate patient care from admission to post-discharge.
• Manage resources and oversee interdisciplinary plan-of-care decisions.
• Participate in the utilization review process and case management documentation.
