Qualifications

• Current clinical licensure strongly preferred.
• One year recent relevant experience required. Acute care setting preferred.
• Demonstrates critical thinking skills and effective communication.
• Ability to work with interdisciplinary team and managed care processes.

Responsibilities

• Works with physicians and multidisciplinary team to develop a plan of care for assigned patients.
• Monitors patient progress and ensures progression towards outcomes.
• Identifies and resolves barriers to effective patient care and participates in discharge planning.
