Job summary

UAB Medicine in Birmingham, AL seeks a Rehabilitation Care Coordinator for Spain Rehabilitation Center to coordinate care services, conduct utilization review, and provide cost-effective case management for rehab patients as they move through the continuum of care.

The role requires a Bachelor’s degree in Nursing/OT/PT/SLP with 3 years of rehab experience, Alabama licensure eligibility, CPR certification, and rehab case management experience preferred.

Qualifications

• Bachelor's degree in Nursing, Occupational Therapy, Physical Therapy or Speech-Language Pathology and three years of related rehab experience.
• Current license or eligibility to be licensed as SLP/OT/PT or RN by Alabama Board.
• CPR certification required or license eligibility.
• Rehab case management experience preferred.

Responsibilities

• Coordinate care services, conduct utilization review and provide case management for Spain Rehabilitation Center patients.
• Orient and support the patient and family to the rehabilitation process.
• Assess patient status and coordinate discharge planning.
• Facilitate transition of the patient through the continuum of care.
